- Do you need assistance finding or navigating services, resources, and/or tools?
- Do you reside in Stark County?
If you answered yes to all three questions, then you may be eligible for our program. The Community Action Pathways HUB (CAPHUB) utilizes Community Health Workers (CHWs) that will help you address your needs to help you on your Pathway to a healthy pregnancy, delivery, and postpartum.
The CAPHUB is a FREE program that provides support, education and lived experience to help connect you to needed services, resources, and tools.
